At Elite Cognition we know that payment decisions for mental and behavioral health services can be personal. Our team wants patients to experience the highest quality of care regardless of financial status. We are happy to help navigate the payment process. We offer the following payment options:
Private Pay
Also called Self Pay or Out of Pocket. When individuals pay for mental health services through self-pay (not utilizing insurance) there is an increase in confidentiality: diagnoses, session dates/times, treatment plans, etc. remain between you and your provider. You also have greater flexibility in treatment decisions, location, length and frequency of session, treatment duration, modality, and more.
All services are to be paid on day of service. We can accept cash, credit/ debit cards, and/or FSA/ FLEX/ HSA/ QSHERA benefit cards. We require clients to have an active card on file.
We offer a sliding scale fee option for those unable to pay our full private pay fee(s) yet wanting to use the self pay option. Rate is determined by application and applied using federal poverty rates to support access regardless of income.
In Network Insurance
We are contracted with and can accept the following state and commercial insurance plans: Quartz, Dean Health Plan by Medica (Dean HMO, SSM Health), Forward Health (BadgerCare, WI Medicaid). Submit a copy of your insurance card at time of referral. We will verify benefits right away. Clients using insurance are responsible for all co-payments, co-insurance, deductibles, and/or payment requirements of their insurance plan on day of service. We can accept cash, credit/ debit cards, and/or FSA/ FLEX/ HSA/ QSHERA benefit cards. We require clients to have an active card on file.
Note: Services and place of service may be dictated by insurance plan.We are not in network with Medicare and cannot accept plans connected to Medicare from the above insurance plans.
Coordinated Care and Service Programs
We are contracted with the following programs: Dane County CCS, My Choice Wisconsin (Molina), IRIS. Submit a copy of your insurance card at time of referral. Pre-authorization is typically required prior to beginning services. We are happy to support coordination with case managers and/or service facilitators, between agencies, to match codes, timing and other nuances.
Note: We are not in network with Medicare and cannot accept plans connected to Medicare from the above insurance plans.
Out-of-Network (OON) & Superbill
Insurance plans that offer Out-of-Network benefits allow clients to see an out-of-network provider (not contracted with their insurance) of their choice for mental health treatment.
At Elite Cognition clients accessing OON benefits pay out of pocket for services. The client then requests a Superbill (itemized receipt) from our agency to submit to their insurance plan for reimbursement. Superbill submission is NOT a process we will do for you. Each insurance company has different rules regarding Out-of-Network benefit reimbursement. It is the client’s responsibility to contact their insurance company, understand the policy, and submit the superbill.
